Aftercare and Transition Team

Welcome to the Dean and Cauvin Trust Aftercare Service and Transition Team!
We have put this information pack together to provide you with all the information you will need in working with the service as well as a number of extras that you might find helpful along the way.

It is our aim is to provide you a service that is as unique as you are and that can respond fully to your support needs and interests, and we look forward to getting to know you.

Aims of our Service

At Dean and Cauvin Aftercare we aim to provide a tailored support service to your individual support needs in a way that values you as an individual. We work with a range of young people, some who have lived in Portland Street and Cauvin House and some that have not.
All young people are allocated a personal case manager from the service as well as a range of Aftercare support staff to ensure that we can support you with many things at the same time. We will work with you to develop an individual support plan that is led by you as we strongly believe that young people are the experts in their own lives, and we are here to support and guide you on your journey into adult life.

The Aftercare Service is available between Mondays – Fridays 9.30am – 4.30pm however we believe in a flexible service that is there when you need it so will make ourselves available to you outside these hours if this suits you better.

We provide 24 hr contact through our residential units at Portland Street and Cauvin House, in case of emergency there telephone numbers are:
