Throughcare to Aftercare

Dean and Cauvin Young People’s Trust provides a dedicated Throughcare and Aftercare service to all young people who are resident within our residential services.

The Aftercare service provides a continuum of care and support, commencing early in the young person’s placement at Portland Street or Cauvin House. Promoting a natural development of a supporting relationship that is well established by the time young people are ready to move on to less supported accommodation.

Each young person has an allocated aftercare worker and initially sees them monthly. The contact increases when young people are coming to the end of their stay and continues once they move into their chosen accommodation.

The service is a partner agency of the City of Edinburgh Council’s Throughcare and Aftercare Service and Barnardos 16+, and provides a holistic range of both practical and emotional supports to young people leaving residential care in a person-centred approach, with specialised support to young parents to develop positive parenting.

We also offer a range of supported tenancies that young people can access when they feel ready to make the transition from residential care, and in addition to individual visiting support the service provides a range of interactive group work programmes that are both fun and educational.

Young people can receive support from the Aftercare Service until they are 26 years of age.
